• Authorized Retailer Application (below), required
• Terms and Conditions, required and sent via email

Once we have both, we’ll review your account within 5 business days. The sooner your forms come in, the sooner you can place your first order. We can only activate accounts (and process orders) once everything is complete, so double-check that nothing’s missing before you hit submit.
